How Self-Advocates Can Join
The Interactive Map 

It's simple and free to participate on The Interactive Map for a person with disabilities or for family members who seek support for their loved ones.

1. To get started, all you do is access the FORM and enter your information.

2. The map will be updated on the Friday that you submit your form to reflect your listing. If a DSP or SDE is available to provide supports for you, they will contact you via the information you share. 

3. We ask that you contact us once you have identified a support person so that we can remove your listing from The Map.
